Comments:
Often there is alveolar pneumocyte hyperplasia adjacent to the interstitial infiltrates of pulmonary Langerhans cell histiocytosis. It may be prominent enough to mimic lepidic adenocarcinoma (previously called bronchoalveolar carcinoma). Note the thickened alveolar septa due to prominent interstitial fibrosis.
