The tumor cells have mild nuclear pleomorphism with occasional small nucleoli and stippled chromatin typical of Fuhrman grade 2 nuclei. This example demonstrates how patchy higher grade nuclei can be and highlights the importance of adequate sampling and careful examination. The Fuhrman nuclear grade of a tumor is judged by the highest grade identified.
