Loss of cellular cohesion (dehiscence) is a common feature of medullary thyroid carcinomas. This may result in focal pseudopapillary pattern where the fibrovascular areas are surrounded by a cuff of tumor cells with empty space in between. In this image, the tumor in the upper left has conventional appearance, whereas the tumor in the lower right has a pseudopapillary pattern.
