The differential diagnosis of Kikuchi lymphadenitis includes Lupus lymphadenopathy, peripheral T-cell lymphoma, Herpes lymphadenopathy, and Cat-Scratch disease.

Cases of Kikuchi lymphadenitis misdiagnosed as malignant lymphoma have frequently been the target of malpractice lawsuits.
