Image Description

The liver may be involved by primary (AL) or secondary (AA) amyloidosis. Both manifest with amyloid deposition in portal vessels and hepatic sinusoids, leading to hepatocyte atrophy. With disease progression (as seen here), the residual liver parenchyma comprises nodules separated by abundant amorphous, lightly eosinophilic amyloid deposits, which can be confirmed by apple green birefringence on Congo Red staining with polarized microscopy.
