1: Urothelial Carcinoma-in-situ
Comments: There is full thickness replacement of normal urothelium by highly atypical cells. However, full thickness change is not necessary for the diagnosis of urothelial CIS, unlike in some other locations.
2: Urothelial Carcinoma-in-situ
Comments: This photomicrograph shows large pleomorphic cells with abundant cytoplasm, oval or irregular nuclei containing one or more nucleoli, and increased mitotic activity. The cells are oriented somewhat randomly producing a disorganized appearance.
3: Urothelial Carcinoma-in-situ
Comments: Occasionally the cells may be small with high N:C ratios. The nuclei are irregular, enlarged and hyperchromatic.
4: Urothelial Carcinoma-in-situ involving Brunns Nests
Comments: Note the involvement of one of the Brunns nests (bottom) by urothelial CIS. The smooth rounded contours help distinguish this from stromal invasion.
5: Urothelial Carcinoma-in-situ
Comments: Tumor cells in urothelial CIS are quite discohesive and are shed in urine in large numbers. Many such shed cells are present in the debris near the surface in this photomicrograph.
6: Urothelial Carcinoma-in-situ : Denuding Cystitis
Comments: In urothelial CIS, cellular discohesion results in shedding of large numbers of tumor cells in the urine. In some cases, the surface contains rare or no recognizable cells – referred to as denuding cystitis. Arch Pathol 96:91-94, 1973.
7: Urothelial Carcinoma-in-situ : Denuding Cystitis
Comments: Higher magnification view of previous focus reveals rare tumor cells among scant remaining urothelial cells.
